For the Family Card, each family member will be entered one at a time. After
you have completed the process for one family member, you will return to the
first screen to add additional family members.
Eligible Family Member - Head of household, spouse and any unmarried dependent children living at home up to age 18 and/or full-time college students under age 24. No charge for children up to 5 years of age as of 12/1/2007. Family members added after 10/31/07 will be charged the higher rate. Proof of age may be required.
